First-Year and Transfer Students

If you are a fall 2020 incoming student, please explore the required tasks tasks below.

By Wednesday, July 1, 2020, your health history and immunization forms must be uploaded to the Patient Portal. For additional information or questions, please contact the Medical Records Office at health@syr.edu or by calling 315.443.2667.

In accordance with New York State law, the following vaccination records must be submitted to the Patient Portal by Wednesday, July 1, 2020:

  • Two Measles, Mumps and Rubella (MMR) vaccinations
  • Meningococcal meningitis vaccination within the last five years and after the age of 16

You may obtain these records by contacting your high school or primary care physician. If you have any questions about your vaccination records, please contact your primary care physician.

By appointment only, Barnes Center at The Arch Health Care is available to assist you in obtaining the appropriate vaccinations if you have not been vaccinated or do not have documentation of your vaccinations. If you have questions or need help using the Barnes Center at The Arch Health Care Patient Portal, please contact Health Care at 315.443.8000 or via email at BarnesCenter@syr.edu.

By Wednesday, July 1, 2020, all new students are required to complete a Tuberculosis (TB) Screening Questionnaire on the Patient Portal.

June Bursar bills will reflect the anticipated premium charge unless an approved waiver is received by Monday, June 1, 2020.

The 2020-2021 Student Health Insurance Plan  enroll/waive form is now available on MySlice. Students must enroll or waive the Student Health Insurance plan by Monday, Sept. 21, 2020.

Each year, all full-time, matriculated undergraduate, graduate and law students are required to demonstrate proof of qualifying health insurance coverage or enroll into the Student Health Insurance Plan. Students within traditionally online degree programs are not eligible to enroll in or waive the Student Health Insurance Plan.

Community Wellness Requirements are centered on sexual and relationship violence prevention, alcohol and other drugs and diversity, equity and inclusion. Please note that failure to complete these requirements will result in a Conduct Hold, hindering class registration in addition to participation in Fraternity and Sorority Affairs, Athletics, studying abroad and more.

  • July 1: Three EVERFI Training Modules (AlcoholEdu, Sexual Assault Prevention, Diversity, Equity and Inclusion)
  • Aug. 1: Virtual Speak About It Presentation

Returning Students
